Intensive Care Coordination Services (ICC)

ICC is a holistic form of care coordination designed to help keep the families of youth with severe behavior issues whole. Our certified Intensive Care Coordinators are trained to utilize the evidence-based High-Fidelity Wraparound Model, meaning that we put the voice of the family first. Our Intensive Care Coordinators create a structured plan to help families meet their self-identified needs and achieve the goals that they envision for themselves.

Who Is this service for?

This service is designed for youth and families containing a youth who has severe behavioral issues and is at risk of, or currently in an out-of-home placement.

When providing this service, our certified Intensive Care Coordinators will:

  • Engage with the youth and family
  • Develop a short-term crisis stabilization plan 
  • Complete a Strengths, Needs, and Culture Discovery (SNCD) Assessment which helps the family identify strengths and family culture, define their needs and goals, and come up with a family vision.
  • Form a youth and family team to identify and carry out action plans. Said action plans are frequently revised and updated, and are driven by what the family wants
  • Complete a Functional Assessment, which identifies potential crisis behaviors in order to better understand the function/purpose of the behaviors as well as what is reinforcing the behaviors. This is used to develop a Crisis Prevention Plan
  • Create a transition plan that incorporates formal and natural supports in the community, and equips families with the tools they need to successfully manage themselves.
